Our laboratory would be happy to set up a standing order from your physician for re-occurring labs. Standing orders are valid for six months, after which time we will need a new test requisition.
Standing orders can be faxed directly to our lab at 509.933.8795.
Standing orders should include
- Patient name
- Ordering caregiver
- Test(s)
- Interval between testing
- ICD-9 diagnosis code
- Physician phone number
- Fax number for results
